Once you and your family have made a commitment and decision to adopt a dog, please download the form and email/fax the completed form to us at info@furKidz911.org.

Filling out the form below is the first step in our adoption process. We will review your application to make sure your family lifestyle and home environment is a good match for the dog you are applying for. The application must be filled out completely before we can schedule a meet & greet with the dog you are applying for.

COPY OF ADOPTION CONTRACT

I/We understand that I am/we are not PURCHASING a dog, but ADOPTING and SAVING THE LIFE of an orphaned dog. Therefore, there is NO TRIAL PERIOD and will be NO refund for ANY reason other than the dog being terminally ill. You (and your family) MUST make certain, this is the dog for you and you are willing to give said dog an unlimited time to adjust to his/her new home.

The dog you are adopting has been surrendered, abandoned, or in some way displaced. I/we understand that there may be behavioral issues, such as hiding, shyness, not eating, growling, diarrhea, accidents, destructiveness and a variety of other issues. Remember to be patient, consistent and understanding. All available information and medical records have been provided to you.

I/We hereby agree to care for this dog humanely (including providing adequate food, water, shelter, love and attention). I/We also agree not to sell, trade or dispose of this dog. If, at any time, I/we are unable or unwilling to care for this dog, I/we agree to contact and return said dog to FurKidz 911 Connection. I must also call FurKidz 911 Connection before returning the dog due to space and availability restrictions.

I/We agree that said dog shall not be used for medical or any other experimental purposes or sold or given to a pet store, supplier or wholesaler. I/We agree that said dog will not be used for any illegal purposes and will NEVER be fought in any way.

I/We do hereby adopt this dog for MYSELF and MY FAMILY (not as a gift or companion for anyone OUTSIDE the home at the address above). I/we understand that this adoption is a LIFETIME commitment and I/we will NOT give up on this newly adopted dog for any reason - other than TERMINAL illnesses or SEVERE aggression, which has been confirmed by a Licensed Vet and/or Professional Trainer or Behaviorist.

I/WE HEREBY ACKNOWLEDGE THAT I/WE HAVE READ AND UNDERSTAND THE ABOVE TERMS AND CONDITIONS AND WILL KEEP THE DOG DESCRIBED HEREIN AS A FAMILY MEMBER. I/WE UNDERSTAND THAT THIS IS A BINDING CONTRACT.

ADOPTION FEES

We are working with limited funds and our rescue runs strictly on adoption fees and donations.

Our fees for adoption are $100 to $400 depending on the situation of the dog or pup. The fee covers only a portion of our expenses (spaying or neutering for dogs older than 4 months, pain medication, 1st set of vaccinations including rabies, deworming, and microchipping). When a healthy dog comes into our rescue, it averages $185 for pre-adoption care expenses. It is not unusual for the cost of vet care for unhealthy dogs to run from $450-$3000 per dog.

Payment Options: Cash, Check, and we are now accepting credit cards (Visa, MasterCard, American Express or Discover).

**Please be advised, if you are paying by credit card, you will be charged a service fee based on the total amount of your transaction. You will be informed of all charges and fees before you authorize the payment. The current rate for each credit card swipe is 2.75%, (3.5% if credit card is entered manually).
